Paperwork... Yes, OE needs it too!
Everyone must complete 2 forms once a year.
*If you are applying for scholarship funding there is a 3rd form.
1. (required once a year) 2012 Medical Form - Everyone must submit a 2012 medical form at least 5 days before the trip(s). This includes moms, dads, personal care attendants and anyone else on a trip. Our staff members even fill these out! The good news is that we only need this form once each calendar year!
2. (required) Participation Form - OE must have your participation form with payment or co-payment and scholarship application to reserve your place on a trip.

3. (optional) 2012 Scholarship Application Form- Scholarship funding is available for children, youth and adults with disabilities and who have financial need. This form only needs to be submitted once a year. Applications for scholarship funding should be submitted at least 10 days prior to the trip for priority consideration.
If you qualify for this funding, a $10 co-pay for each trip you want to attend must be submitted with the 2012 Participation Form. People without disabilities are ineligible for scholarship funding.
If you are unable to pay the co-pay, contact Dave right away. We have limited special circumstance co-pay funding for certain trips. To qualify for this funding we will need complete household income documentation.
Scholarship award notifications will be made as soon as possible and no later than three days prior to each trip. For more specific Information call 781-395-4999 or email your questions to Dave, our money guy - dave@outdoorexp.org.
